REQUESTING YOUR MEDICAL FILE

Article L.1111-7 and R.1111-2 to R. 1111-9 of the Public Health Code / Order of 5 March 2004 modified by the order of 3 January 2007 / Decree of 4 January 2006
Information about your state of health is compiled in a medical file, which is compiled when you are admitted to our departments. You can access this information by making a request to the management of the establishment.

You can access your file :

During your stay: you must make your request orally or in writing and make an appointment with the department concerned. If you wish, a doctor will be able to answer your questions.
After your hospitalisation: please download, print and complete this form and send it by post to Clinique de l'Alma - Secrétariat de Direction - 166, rue de l'Université 75007 Paris or by email to assist.dir.alma@almaviva-sante.com. Please also download the list of documents to be provided.
If you wish to obtain a copy of all or part of the information in your file, you will be responsible for the cost of reproduction and postage (25 euros). Finally, in accordance with current regulations, your medical records are kept for twenty years from the date of your last stay or your last outpatient consultation (except in special cases).
